Do you love helping children and families thrive TFI Family Services is hiring a full-time Aftercare Case Manager to join our team!
What Youll Need
- A bachelors degree in human services.
- A valid drivers license.
- A willingness to learnwell provide all the training you need!
A Day in the Life
As an Aftercare Case Manager youll support children and families who have achieved permanency through reintegration adoption custodianship/guardianship or aging out of care. Youll:
- Focus on risk management and help families maintain stability during the aftercare period.
- Provide crisis intervention and work with public and private agencies offering supportive services.
- Maintain accurate documentation and reports.
- Coordinate community and educational services for families.
- Meet with families in-person to support reunification and resolve any concerns.
