What does the role involve and WHY you will play a huge part in peoples lives.
- Deliver person-centred care in clients own homes
- Assist with personal care (washing dressing toileting continence)
- Support with mobility transfers and safe use of equipment
- Administer or prompt medication in line with care plans and MAR charts
- Prepare meals and support nutritional needs and hydration
Carry out light household tasks to maintain a safe clean environment - Provide companionship and emotional support to reduce isolation
- Observe monitor and report changes in health or behaviour
- Follow individual care plans and promote independence and dignity
- Maintain accurate daily records and care notes
- Adhere to safeguarding health & safety and infection control procedures
- Communicate effectively with families healthcare professionals and supervisors
- Support clients with appointments shopping and community access
- Respect confidentiality and comply with GDPR and CQC standards
