Job Summary:

The Registered Nurse provides skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. This role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Strong communication and collaboration skills are essential for effective interaction with patients providers and team members.
Location:Pittsfield Massachusetts United States
Responsibilities:

- Provide skilled nursing care to patients.
- Conduct patient assessments and develop treatment plans.
- Administer medications and treatments as prescribed.
- Collaborate with the interdisciplinary team to ensure comprehensive patient care.
- Maintain accurate and timely documentation of patient care.
- Communicate effectively with patients families and healthcare providers.
Required Skills & Certifications:

- Active Registered Nurse license in Massachusetts.
- Bachelors degree in Nursing or related field.
- Minimum of 2 years of recent Medical/Surgical experience.
- NIHSS certification.
- BLS certification.
- ACLS certification.
Preferred Skills & Certifications:

- Specialities in ICU ER Psychiatric Labor & Delivery PACU or Operating Room.
- Experience in case management or hospice care.
Special Considerations:

- Nights 7p-7:30a shift.
