Licensed Social Worker (LICSW or LCSW)

About the Role:

Focus exclusively on the geriatric population
Conduct psychosocial assessments and create individualized care plans
Participate in daily interdisciplinary team meetings
Coordinate facility admissions and discharges as needed
Educate and support families with community resources

Qualifications:

Minimum of 1 year of experience in Social Work providing traditional clinical or case management services with a geriatric population
Active LICSW or LCSW license required
Strong communication and teamwork skills
Compassionate approach to supporting older adults and their families
