The Care Manager III (CMIII) provides intensive case management services for a caseload of up to 20 high risk Health Plan of San Clara members.
The CMIII is responsible for the assessment of clients with multiple medical and psychosocial needs. The CMIII also plans monitors services and interventions ensuring provision of quality care and provides mentorship and training for all new case management staff and interns.
  • Conducting comprehensive assessments and re-assessments of client psychosocial physical and mental health environmental and spiritual needs to develop the client Community Living Plan.
  • Conducting on-going client home acute hospital and skilled nursing facility visits to review and modify their Community Living Plan.
  • Providing crisis intervention advocacy problem solving and therapeutic interventions services to the client.
  • Writing comprehensive assessments.
  • Based on assessment information with the client develops and initiates the Community Living Plan which is client-centered comprehensive and consistent with program guidelines and policies and procedures.
  • Documenting and updating progress notes of all case management activity within 24-48 hours.
  • Evaluating client ability to remain safely at home and coordinate appropriate placement if needed.
  • Acting as liaison between the client caregiver physicians and community service providers to update arrange and monitor client care-based program operation guidelines. Providing mentorship consultation and training to all new case management staff and interns.
  • Attending and participating in program leadership research study continuing education and Care Manager Development Program meetings as required.

REQUIRED QUALIFICATIONS:

  • Masters degree in Social Work (M.S.W.)
  • 2 years of experience working with disabled adults and/or older adults.
  • 2 years of experience resolving medical and psychosocial problems for functionally impaired adults.
  • Exceptional communication and presentation skills.
  • Detail oriented with good problem-solving skills and the ability to prioritize multiple tasks.
  • Proficient computer literacy skills.

DESIRED QUALIFICATIONS:

  • LCSW certification.
  • 1 year of experience working with individuals with mental and/or behavioral health diagnoses and substance abuse disorders
  • Bilingual language skills.

COMPENSATION

Range: $95170 - $113742/Annual

This amount is not necessarily reflective of actual compensation that may be earned nor a promise of any specific pay for any specific employee which is always dependent on actual experience education and other factors.
