Great part-time (2 days per week) primary care role managing complex patient cases in a post-acute setting (SNF) in West Chester Pennsylvania focusing on improving care for some of the most vulnerable patients.

- Active and unrestricted Pennsylvania license (or in process)
- Active and unrestricted DEA/prescriptive authority (or in process)
- A deep commitment to improving patient outcomes leading clinical excellence and driving change in the healthcare landscape
- Internal medicine (IM) family medicine (FM) emergency medicine (EM) hospitalist or candidates with any related experience are welcome to apply
