Job Summary

Post-Acute Care Physician (Part Time) opportunity in West Chester PA

Great part-time (2 days per week) primary care role managing complex patient cases in a post-acute setting (SNF) in West Chester Pennsylvania focusing on improving care for some of the most vulnerable patients.

Qualifications:

Minimum of 1 year of experience with a commitment to delivering high-quality patient-centered care
Post-acute care experience is beneficial though not required as we provide comprehensive support and development opportunities
Active and unrestricted Pennsylvania license (or in process)
Active and unrestricted DEA/prescriptive authority (or in process)
A deep commitment to improving patient outcomes leading clinical excellence and driving change in the healthcare landscape
Internal medicine (IM) family medicine (FM) emergency medicine (EM) hospitalist or candidates with any related experience are welcome to apply
