Eist is multi-disciplinary team aligned to Southside Intellectual Disability Services and Adult Community Disability Services IHADSW.
A panel may be created from this campaign for Clinical Nurse Specialist (Intellectual Disabilities Health and Wellbeing) HSE Dublin South & Wicklow from which current and future permanent and specified purpose vacancies of full time or part time duration may be filled.
Purpose of the Post:
The purpose of this Clinical Nurse Specialist post is to deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse / Midwife Specialised Posts 4th Edition National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008. The CNS will:
Provide a specialis psycho social holistic health and well-being Intellectual Disabilities service to residents and service users of SSID and Adult Community Disability Services their families and/or carers.
Provide specialist nursing advice to Health and Social Care Staff support families and carers.
Implement evidence based practice to individuals with an Intellectual Disability and/or autism.
Provide holistic nursing assessment and management including health screening health promotion and chronic disease management.
Collaborate with the MDT for the co-ordination management and leading in the development and delivery of services for adults with disabilities and / or autism within South Side Intellectual Disability and Adult Disability Services in Dublin South Wicklow
Be required to support the principle that the care of the service user comes first at all times and will approach their work with the flexibility and enthusiasm necessary to make this principle a reality for every resident to the greatest possible degree.
Maintain awareness of the primacy of the individual in relation to all service activities.
The post holder will liaise with the person their keyworker wider staff support and the MDT in order to design and support any plans put in place for each individual. The focus will be person centred and proactive.
The caseload for the post holder will be adults with intellectual disability/and or Autism who are currently using South Side Intellectual Disability Services and Integrated Health Area Dublin South Wicklow Disability Services.
Clinical Nurse Specialist in Intellectual Disabilities Health and Wellbeing will initially focus on the adults referred for interdisciplinary supports via the Central Referral Committee/ÉIST referral pathway.
