Job Overview
A dedicated and experienced Licensed Practical Nurse (LPN) is needed for a long-term care assignment in Montour Falls NY. This role is ideal for a compassionate clinician who is committed to providing high-quality patient care in a supportive environment. The assignment is 13 weeks with a strong likelihood of extension based on performance and facility needs.
Key Responsibilities
Provide patient-centered nursing care in accordance with established care plans.
Conduct assessments and develop individualized care plans based on patient needs and medical conditions.
Prioritize and deliver nursing interventions aligned with clinical best practices.
Monitor document and report patient conditions accurately and promptly.
Ensure all documentation meets clinical standards and regulatory requirements.
Collaborate with interdisciplinary team members to deliver holistic coordinated care.
Promote evidence-based practices and participate in continuous improvement initiatives.
Adhere to all health safety and infection control regulations.
Perform additional duties as assigned within scope of practice.
Requirements
Experience
Minimum 2 years of recent LPN experience in long-term care or a similar clinical environment.
Licensure & Certifications (Required Before Start)
Active New York LPN license
AHA BLS certification
