Job Summary
Post Acute Care Physician opportunity in Endicott NY (Part Time)
Excellent part-time (2 days per week) primary care role providing patient care in a post-acute care setting in a skilled nursing facility (SNF) located in the Endicott New York area. You will round 2 days a week with the flexibility to choose your rounding schedule.
Position Highlights:
- Primary care rounding in skilled nursing facilities
- Manage chronic disease diagnose illnesses and medical conditions and implement plan of care
- Collaborate with clinicians facility leaders and others regarding care and services
- Build relationships with your patients patients family members and facility staff to facilitate effective treatment delivery
- Provide oversight guidance and mentorship to our nurse practitioners
- Conduct clinical case reviews as needed and share tools information and guidelines as they relate to quality of care
- Collaborate with leadership to review care coordination initiatives develop collaborative intervention plans and share ideas about network management issues
- Minimum of 1 year primary care experience
- Post-acute care experience a plus
- Active and unrestricted New York license or application in process
- Active and unrestricted DEA/prescriptive authority or application in process
