Provides transplant social work services for facilitating solutions to the medically related psychosocial/environmental problems of patients and families. Ensures the continuity of patient care by implementing interventions which may include initial assessment identifying mental health disorders brief behavioral health interventions information and referral during hospitalization and/or outpatient treatment. May provide educational support to staff. Maintains positive working relationships with vendors and external agencies to access services for patients. The individual must possess the ability to clinically assess and treat emotional mental health issues.

Job Description

Ensures the continuity of patient care by implementing interventions which may include initial assessment identifying mental health disorders brief behavioral health interventions information and referral during hospitalization and/or outpatient treatment. May provide educational support to staff. Maintains positive working relationships with vendors and external agencies to access services for patients. The individual must possess the ability to clinically assess and treat emotional mental health issues. Masters degree in Social Work. Minimum of one year of current experience in a clinic hospital medical setting or health care field. This position will cover all solid organs but is primarily caring for the liver transplant patient population. Leadership experience and experience with transplant patients or dialysis patients preferred.

Additional Job Description

Education: Masters degree in a social work or social welfare program accredited by the Council on Social Work Education. Experience: Five years post graduate professional work experience in social work : MSW licensure by the South Carolina Board of Social Work Examiners. Certified ACM/CCM LISW or PHD in related field preferred.
