Job Summary and Responsibilities

****Good Samaritan Hospice Indiana - Illinois -Hiring Fulltime Medical Social Worker****

****New Pay Rates - Competitive Benefit Package- Mileage Reimbursement -Tuition Reimbursement****

****5 Days (8:00 AM - 5:00 PM) hours Per Week- No Weekends or After Hour On Call requirements**

At Good Samaritan Hospice Indiana -Illinois we strive to embody our mission of delivering hope and healing to those we serve. As a Medical Social Worker youll engage in a range of exciting responsibilities including: Service Areas include: Southwestern Indiana Counties of Davies Gibson Greene Kox Martin Pike and Sullivan. Illinois Counties Lawrence Wabash Crawford and Richland.

  • Establishing a care plan in accordance with the physicians plan of treatment.
  • Providing crisis intervention as required.
  • Assisting the physician and other caregivers in understanding the significant social emotional and economic factors influencing the patients ability to improve medically.
  • Providing patient care education to patients and caregivers.
  • Assisting in making appropriate community referrals to gain access to additional resources as needed (i.e. applications for public assistance housing problems additional health care resources transportation)
  • Working with the patients caregivers to resolve problems interfering with an optimum caring relationship between the patient and caregiver.
  • Completing and submitting all required documentation in an accurate and timely manner.

Job Requirements

  • Masters degree from an accredited school approved by the Council on Social Work Indiana must also be certified under IC25-23.6-4-3.
  • Currently licensed to practice in the state serviced.
  • One year social work experience in a health care setting.
  • Current CPR/BLS certification through American Heart Association or American Red Cross and compliance with state requirements
