Description
The Verland Admission Coordinator serves as the primary point of contact for all referrals to Verlands Intermediate Care Facilities (ICF) Community Living Arrangements (CLA) and Adult Training Facilities (ATF). This role ensures timely and coordinated admissions facilitates communication between internal departments and external agencies and supports families and guardians throughout the referral and admission process.
Schedule:
Monday - Friday: 8:30am-5pm
Contributions:
- Serve as the central coordinator for all incoming referrals to ICF CLA and ATF programs.
- Manage the admissions pipeline including intake documentation eligibility verification and coordination of pre-admission assessments.
- Chair the Admissions Committee and present referral cases with relevant health and support documentation.
- Liaise with families guardians service coordinators and county representatives to ensure smooth transitions into Verland programs.
- Schedule and conduct site visits and tours for prospective individuals and their support teams.
- Collaborate with clinical and habilitation teams to ensure readiness for new admissions.
- Maintain accurate records of all referrals admissions discharges and transfers in the electronic health record system.
- Ensure compliance with federal state and agency regulations regarding admissions and eligibility.
- Provide regular updates to leadership on referral trends admission status and capacity planning.
- Participate in multidisciplinary meetings to support individualized planning and integration.
- Respond to inquiries from external agencies and families regarding program availability and admission criteria.
- Support marketing and outreach efforts to promote Verlands programs and services.
Requirements
- Strong knowledge of IDD services including ICF CLA and ATF program structures.
- Excellent communication and interpersonal skills.
- Ability to manage multiple priorities and maintain detailed records.
- Familiarity with HIPAA and confidentiality standards.
- Proficiency in electronic health records and Microsoft Office Suite.
- Ability to work independently and collaboratively across departments.
Minimum Training and Experience:
- Bachelors degree in Social Work Human Services or related field required.
- Masters degree preferred.
- Minimum of two years of experience in admissions coordination or case management in IDD services.
- Valid drivers license and reliable transportation.
- Required clearances: Act 34 Child Clearance Act 34 Criminal Clearance Act 73 FBI Clearance.
