Overview
Provides assessment support information and referral advocacy services and other resource assistance to patients and their caregivers directly or through consultation within a multi-disciplinary outpatient team.

Responsibilities
- Provides support to pediatric patients and families in an outpatient clinic setting as part of a multidisciplinary team to help address psychosocial needs within the context of the patients medical care.
- Completes psychosocial assessments of pediatric patients and families to assess current supports strengths areas of need and barriers to accessing care.
- Coordinates and connects patients and their caregivers with community resources to address/meet identified needs. Serves as a link between the medical team and the patients community/natural supports.
- Assists caregivers in adapting to difficult and complicated dispositions involving complex care needs multiple agencies/providers and linkage to resources in the community.
- Provides consultation assessment referral and coordination in cases of suspected child abuse or neglect.
- Organizes support groups provides psychoeducational materials and opportunities for patients and their caregivers based on their individual medical needs.
- Documents objective concise and pertinent clinical information in the medical record.
- Perform other duties as assigned.
Qualifications
- Masters Degree in Social Work (MSW) required.
- Social work experience in an acute home care or ambulatory setting preferred.
- Strong organizational skills verbal and written communications skills.
- Sound decision making and judgment.
- Time management skills.
- Negotiation skills.
- Proven ability to deal with conflict resolution.
- Comfortable with basic computer skills.
