Job Summary
MUST have a clear driving record active driver s license and current auto insurance. Must provide proof within 48 hours of offer acceptance!
6 patients per day/Productivity of 30 per week
Must complete revisits SOC/ROCs recerts and discharges
Mileage reimbursed at $0.62 cents per mile

Requirement description :
- 1 years of recent Home Health experience - REQUIRED
- Case management experience - REQUIRED
- Homecare Home Base EMR - REQUIRED
- Previous travel experience - Highly Preferred
- IV and venipuncture experience - Highly Preferred

- 2 supervisory references from the last 2 years

- Active License (verification must be less than 30 days old)
