Facility Reimbursement for the Following Costs: Mileage Billed at the IRS Standard Rate

Requirement description :

Board Certified - REQUIRED
Active VA license (no exceptions) - REQUIRED
BLS ACLS - REQUIRED
Provider Must be Local - Only Mileage is Reimbursed - REQUIRED
Provider must submit a flat rate for 24 hour coverage to include ALL gratis hours to be considered
