Shift & Schedule
- Shift: Days
- Schedule: Tuesday Saturday
- Weekly Hours: 40 hours
- Daily Hours: 8 hours
- Start Time: Between 8:00 AM 9:00 AM
- Shifts per Week: 5
Position Requirements
- Education:
- Masters Degree in Social Work (MSW) Required in CA
- Experience:
- Minimum 1 year experience in an acute or long-term medical setting Required
- 2 years experience in an acute or long-term medical setting Preferred
- Must have acute care experience
- Licensure:
- Active Social Work License in CA
- Additional Notes:
- Must start ASAP
- Looking for a true Social Worker not a Case Manager
Job Description
The Social Worker provides professional services to patients and their families to address needs arising during hospitalization. Responsibilities include:
- Offering counseling emotional support and guidance
- Assisting patients and families in developing transition and discharge plans
- Coordinating with outside agencies nursing staff and family members
- Addressing non-medical concerns and supporting patient wellbeing
