Overview
Health Care Connectors is seeking a compassionate and skilledHome Health Nurse Practitionerto join our growing clinical team. The ideal candidate is clinically strong organized and passionate about delivering high-quality patient-centered care in the home setting.
Key Responsibilities
- Perform comprehensive patient assessments in the home including physical exams medication reviews and chronic disease management.
- Act as theprimary care providerfor patients who do not currently have a PCP ensuring continuity of care and ongoing clinical oversight.
- Complete and maintainOASIS documentationaccurately and on time (required).
- Develop implement and evaluate individualized patient treatment plans.
- Collaborate with RNs therapists physicians and other interdisciplinary team members to coordinate care and improve patient outcomes.
- Provide education to patients and families to support disease management and promote independence.
- Participate in in-office case conferences training and team meetings (approx. 25% of workload).
- UtilizeHomecare Homebase (HCHB)for documentation and workflow (preferred).
- Ensure compliance with all federal state and agency standards and regulations.
- Provide on-call support if needed (optionalcustomize as needed).
Qualifications
- Active unrestrictedNurse Practitioner license in Connecticut (CT).
- National certification (FNP AGNP or equivalent).
- Minimum12 years of clinical experience; home health experience strongly preferred.
- OASIS experience required.
- Homecare Homebase (HCHB)experience preferred.
- Excellent clinical judgment communication and organizational skills.
- Valid drivers license reliable vehicle and ability to travel throughout the Connecticut service area.
- Ability to work independently in a field-based environment.
