A panel may be formed as a result of this campaign for Clinical Nurse Specialist (CNS) Neuro-Rehabilitation Community Neuro-Rehabilitation Team HSE Dublin South & Wicklow from which current and future permanent and specified purpose vacancies of full or part-time duration may be filled.

Purpose of the Post:

The purpose of this Clinical Nurse Specialist (Neuro-Rehabilitation) post is to:

Deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse Specialist Posts 4th edition National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008.

The successful candidate will work as a key member of the multidisciplinary Community Neuro-Rehabilitation Team providing a person centred quality service to service users / clients and this specialist practice will encompass a major clinical focus which comprises assessment planning delivery and evaluation of care given to the service user / clients and their families.

The Clinical Nurse Specialist (Neuro-Rehabilitation) together with the interdisciplinary team will provide a specialist neuro-rehabilitation service for persons with acquired and progressive neurological conditions. The CNS will act as a liaison between Community Neuro-Rehabilitation Services Primary Care Community services and other agencies. The appointment of the CNS is an essential element in the provision of effective high quality integrated care for those with neuro-rehabilitative need. The successful candidate will work as a key member of the multidisciplinary Community Neuro-Rehabilitation Team providing a person centred quality service to those under the care of the Community Neuro-Rehabilitation Team. This specialist practice will encompass a major clinical focus which comprises assessment planning delivery and evaluation of care given to the individual and their families.

As a member of the Community Neuro-Rehabilitation Team (CNRT) the post holder will be responsible for planning developing co-ordinating and evaluating the service to meet the needs of individuals and the objectives of the CNRT. The post holder will participate in the development and monitoring of services in conjunction with the team. The post involves working autonomously and as part of an interdisciplinary team within a community setting.

Caseload

The Clinical Nurse Specialist (Neuro-Rehabilitation) will focus on service users under the care of the Community Neuro-Rehabilitation team. The post holders practice is based on the five core concepts of Clinical Nurse Specialist (Neuro-Rehabilitation) role as defined by the NCNM 4th edition (2008) in order to fulfil the role.

This is an exciting opportunity to be part of an inter-disciplinary Community Neuro-Rehabilitation Team in CHO 6. This is a fulfilling role with team members being part of a multi-professional team providing specialised intensive rehabilitation to those with neuro-rehabilitative needs as close to home as possible.

The CNRT will provide specialist community-based Neuro-Rehabilitation. Key to the success of the CNRT will be an individual health professionals ability to provide discipline-specific expertise and care whilst working in an autonomous manner. Given the specialist nature of service delivery in the CNRTs ongoing learning education and robust supervision structures are of paramount importance. Candidates must have sufficient specialist knowledge and expertise in the area of Neuro-Rehabilitation. As such bespoke recruitment campaigns are required in order to ensure the specialist level of care is delivered by these teams. There should be opportunity for candidates from abroad to apply for these posts and to have virtual interview options available.

Background Context to Neuro-Rehabilitation Strategy

As part of the National Strategy and Policy for the Provision of Neuro-Rehabilitation Services in Ireland Implementation Framework 2019-2021 the HSE are developing networks of Neuro-Rehabilitation Services. The overarching aim of the Strategy is the development of Neuro-Rehabilitation services to improve outcomes for people by providing safe high quality person-centred Neuro-Rehabilitation at the lowest appropriate level of complexity. This must be integrated across the care pathway and provided as close to home as possible. These services are to be configured into population based Managed Clinical Rehabilitation Networks (MCRNs). The new model would see the introduction of a multi-tiered system with access to services based on clinically assessed need. This means that instead of people being referred to a single service they are now referred to a network of services and can access services within the network based on their need. All referrals to the network are received on a standardised referral form and are processed through a single point of entry through the Network Rehabilitation Coordinator role. Services work together across organisational boundaries with people moving across the continuum of care seamlessly based on their needs. Within the MCRN the Community Neuro-Rehabilitation Team (CNRT) provides a critical link in the care pathway by facilitating early discharge and continuity from both acute and post-acute rehabilitation facilities.

Description of the Community Neuro-Rehabilitation Team (CNRT) Service

The CNRT is an inter-disciplinary therapy team providing a specialised service for adults with a range of progressive or acquired neurological conditions. This is a multi-professional team including a Consultant in Rehabilitation Medicine Physiotherapy Occupational Therapy Speech and Language Therapy Psychology Social Work Nursing and Administrative staff. This team will provide assessment and intensive therapy to people in both their own homes and in a clinic-based setting for up to a period of 12 weeks. This service will provide people with earlier access to specialist rehabilitation close to home. The team will provide a person centred service improving peoples quality of life and working together to support people with neuro-rehabilitative needs.

This MCRN will bring together tiered levels of specialist Neuro-Rehabilitation services promoting transparent equitable access to neuro-rehabilitation services. Each MCRN has a

Network Rehabilitation Coordinator that acts as the single point of entry for all referrals into the network. CHO 6 is part of the demonstrator project for the National Neuro-Rehabilitation Strategy and the current MCRN in CHO 6 has been live since April 2023 with access to complex specialist tertiary Neuro-Rehabilitation services in the National Rehabilitation Hospital (NRH) and regional specialist inpatient Neuro-Rehabilitation service in the Royal Hospital in Donnybrook. All referrals are received by the Network Rehabilitation coordinator on a standardised referral form. Each referral is directed to the service within the network that best meets the needs of the individual. With the development of the Community Neuro-Rehabilitation Team this network will be complete with all tiers of Neuro-Rehabilitation as per the MCRN model. The National Neuro-Rehabilitation Strategy will continue to engage with key stakeholders in the region to continue to expand existing MCRN services going forward. This is an exciting opportunity as members of the CNRT in CHO 6 will be directly involved in the establishment of one of the first complete MCRNs in the country. CNRT team members will play a significant role in respect to evaluation of the service and demonstrating effectiveness. They will also be supported to contribute to further development of the team and the wider MCRN. Engagement with governance structures such as the Local Implementation Groups (LIGs) and Neuro-Rehabilitation Steering Group (NSG) will also be required

The MCRN and CNRT will sit within a wider health context operating alongside other domains of service such as Primary Care Chronic Disease services Older Persons services and Disability services. The National Neuro-Rehabilitation Strategy recognises that for the majority of people with neuro-rehabilitative need the time in which they are engaged with network services is minimal when compared to their lifespan living in the community with neuro-rehabilitative need. A key priority of the National Neuro-Rehabilitation Steering Group (NSG) is to ensure that MCRNs and services within including the CNRTs are aligned and integrated with existing community Neuro-Rehabilitation services to ensure a streamlined journey for people with neuro-rehabilitative needs. CNRT teams members will be supported to work closely with colleagues within network services and with wider community rehabilitation colleagues.

A culture of life-long learning and development is nurtured in the Health Service. The CNRT post holders will be provided with supports to enhance professional development including time off to study and attendance at relevant courses- access to online training and our leadership academy. Given the specialist nature of service delivery in the CNRTs ongoing learning education and robust supervision structures are of paramount importance. MCRN services are specialist Neuro-Rehabilitation services and there will be robust network education structures established through the National Neuro-Rehabilitation Strategy to ensure that there is cross learning within and across services and MCRNs nationally. Individuals career journey will be supported too with ongoing opportunities to progress into more senior roles. The training and development programme coupled with coaching mentoring and performance achievement allow team members to expand and develop their skill set and competence.
