Through individualized service planning case management health home coordination and peer support the PRP helps clients navigate housing employment education and health care access. Services are person-centered trauma-informed and culturally responsive with an emphasis on whole-person care and long-term wellness. The program serves both youth and adults working in close collaboration with clinical providers families and community partners.

ACCOUNTABLE TO: Psychiatric Rehabilitation Specialist (Operations Manager 3)


ACCOUNTABLE FOR: Delivering billable Medicaid-compliant psychiatric rehabilitation services that promote independence develop life skills and support client treatment goals.


WORK SCHEDULE: Flexible scheduling based on client availability Medicaid requirements and assigned service authorizations. Evenings and weekends may be required depending on caseload needs. DSPs must maintain consistent availability aligned with client needs.


CLASSIFICATION: Fee-for-Service (FFS) W-2 employee


COMPENSATION RANGE: Compensation for this position ranges from $15.00 to $18.00 per hour and is commensurate with experience expertise verified credentials and available company budget.

ANTICIPATED TRAVEL: Travel required within Waldorf Maryland and nearby areas to meet clients in the community. No additional travel compensation is provided.


CLIENT SERVICE LOCATION EXPECTATION:

This position provides community-based PRP services throughout Waldorf Maryland and surrounding service areas as assigned. Services may be delivered in:

  • The clients home
  • Community locations such as libraries parks community centers offices or other public settings
  • Approved community areas appropriate for PRP service delivery
  • Telehealth only when explicitly approved by Medicaid and agency policy (if applicable)

The DSP is expected to travel within the service region meet clients at appropriate community sites and complete documentation in accordance with PRP protocols. This is not a facility-based role.


CASELOAD MAKEUP:

The assigned caseload for this Fee-for-Service (FFS) PRP position consists exclusively of community-based clients living throughout Waldorf Maryland and nearby areas. All services are delivered directly in the community in the clients home or in approved public settings consistent with Medicaid regulations.

Clients will fall under two categories based on their service enrollment:

  • Category B: Clients receiving both Therapy and PRP services
  • Category E: Clients receiving Therapy only but eligible for PRP intake and engagement

Caseload size will vary depending on client needs approved authorizations service utilization and assigned billable expectations. All clients are served in the community and not at a fixed facility or office location.


SUMMARY OF POSITION RESPONSIBILITIES:

The Direct Support Professional (DSP) in the Fee-for-Service Psychiatric Rehabilitation Program (PRP) provides community-based skill-building services to individuals with mental health conditions. DSPs assist clients with developing life skills accessing resources completing recovery-focused goals and improving independence. Services are delivered according to each clients Individualized Rehabilitation Plan (IRP) and documented in Medicaid-compliant progress notes.


SCHEDULED DUTIES AND RESPONSIBILITIES:

  • Provide one-on-one skill-building sessions in alignment with each clients IRP.
  • Assist clients in building Activities of Daily Living (ADLs) and Instrumental ADLs including transportation skills budgeting self-care communication and household management.
  • Support clients with navigating public systems (benefits housing medical transportation employment).
  • Deliver billable PRP services according to client availability and approved service authorizations.
  • Promote client empowerment self-advocacy and recovery principles.
  • Submit accurate timely progress notes in the Electronic Health Record (EHR).
  • Complete documentation in real time or before the end of each service day.
  • Use correct service codes for all PRP sessions:
    • PRP Group off-site (H2016-15)
    • PRP Group on-site (H2016-52)
    • PRP Individual off-site (H2016-15)
    • PRP Individual on-site (H2016-52)
  • Maintain Medicaid minimum service delivery requirements:
    • Minors: Minimum 3 PRP sessions per month
    • Adults: Minimum 6 PRP sessions per month
  • Follow all Scheduling Department procedures and communicate availability for client scheduling.


UNSCHEDULED DUTIES AND RESPONSIBILITIES:

  • Respond to emerging or urgent client needs.
  • Participate in trainings supervision and interdisciplinary team meetings.
  • Assist clients during urgent community needs (housing crises legal matters emergency appointments).
  • Support engagement efforts including wellness events and outreach.
  • Report client safety concerns immediately to supervisors.
  • Maintain HIPAA confidentiality and trauma-informed practices.
  • Collaborate weekly with the clients therapist and RN case manager and submit a collaboration note in the EHR.


PHYSICAL DEMANDS:

  • Frequent walking driving entering community buildings climbing steps
  • Ability to lift up to 20 lbs (e.g. materials supplies)
  • Use of PPE as required
  • Ability to remain mobile in community settings

LEVEL OF EDUCATION / TRAINING / QUALIFICATIONS:

  • High school diploma or GED required. For those working with minors at least a bachelors degree is required.
  • At least 1 year of experience working in behavioral health social services or human services preferred
  • Must pass background check and complete PRP observation by supervisor of 20 hours and 40 hours of training
  • CPR/First Aid certification required (or must be obtained within 30 days of hire)
  • Valid drivers license and reliable transportation strongly preferred
  • Must be able to complete and submit timely accurate progress notes using required PRP service codes
