Job Summary
The Mental Health Association of Monmouth County seeks a full-time Case Manager for its Housing Supports Program. The Case Manager is responsible for the overall case management of Managed Care Organizations (MCOs) members seeking housing security. The Case Manager ensures individuals and families achieve permanent housing stability and improves their overall quality of life reducing reliance on emergency or temporary housing solutions.
The Housing Supports Program Case Manager reports to the Housing Supports Program Manager.
What You Will Do
- Provide overall case management to individuals seeking housing stability and serving as a liaison with service providers.
- Conduct comprehensive needs assessments to identify housing barriers.
- Develop and implement individualized housing stabilization and crisis plans to support housing acquisition and long-term stability.
- Assist individuals in identifying applying for securing and maintaining appropriate housing.
- Collaborate with coordinated entry systems landlords housing providers and community partners to obtain and sustain housing opportunities.
- Connect individuals to community resources including employment programs financial assistance healthcare food clothing and transportation.
- Educate individuals on tenant rights and responsibilities budgeting conflict resolution and life skills needed for housing stability; advocate to ensure fair treatment and access to services.
- Maintain accurate case records attend supervision and staff meetings and represent the agency at community events as needed.
Required Qualifications & Skills
- Two years of related experience.
- Recognize clinical crises requiring emergency intervention.
- Cultural sensitivity and effective engagement with diverse populations.
- Effective verbal and written communication.
- Valid drivers license.
Preferred Qualifications & Skills
Education
- Associates degree in social work or a related human services field.
- Additional experience may be considered in lieu of formal education.
