Job Summary

Weekly hours: 33 to 40 hour per week Monday through Friday day schedule.

Wage range: $18.50 to $20. Full time benefits and mileage reimbursement.

Must have a valid drivers license auto insurance and a clean driving record.

SUMMARY / OBJECTIVE:

Provide direct person-centered support and services to individuals with intellectual and developmental disabilities (IDD) primarily in the community. Represent PAI externally create and facilitate community-based activities foster partnerships and ensure individuals are engaged and supported in meaningful inclusive experiences. This position may start and end in the community and involves minimal office-based work.

ESSENTIAL FUNCTIONS AND PRIMARY RESPONSIBILITIES Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  1. Plan and implement inclusive community-based activities aligned with participant preferences and support plans.
  2. Provide individualized support and supervision in community settings such as libraries parks recreational centers volunteer sites and local businesses.
  3. Deliver Individualized Home Supports (IHS) services as needed.
  4. Represent PAI to community members businesses and partner organizations in a professional and positive manner.
  5. Build and maintain relationships with community partners to expand opportunities for participants.
  6. Provide transportation using a PAI or personal vehicle (mileage reimbursed) including coordinating start and end locations as needed.
  7. Promote self-advocacy independence and community engagement for each person supported.
  8. Promptly complete documentation of service delivery outcome progress and community engagement.
  9. Maintain confidentiality and support participant rights at all times.
  10. Assist in adapting activities to meet the needs of individuals with physical or sensory needs.
  11. Monitor safety and provide behavioral support consistent with individualized plans.

SECONDARY RESPONSIBILITIES

  1. Attend and contribute to staff meetings trainings and planning sessions.
  2. Assist with occasional onsite responsibilities or center-based programming as needed.
  3. Stay current on best practices for community integration and disability services.
  4. Support other team members in coordinating or covering services.
  5. Complete mandatory trainings including person-centered planning medication administration (if applicable) and safety/emergency procedures.
  6. Other duties as assigned.

JOB SPECIFICATIONS

Education Experience and Credentials

Required

  1. At least 5 years of direct care experience with individuals with disabilities.
  2. High school degree or G.E.D. and be 18 years of age or older.
  3. Pass DHS background study and maintain clearance to provide direct services without continuous supervision.
  4. Reliable transportation for work driving purposes valid drivers license proof of valid car insurance satisfy insurance criteria for driving history and permit driving record checks as needed.
  5. Comfortable with transporting individuals with disabilities in your own vechicle or company owned van.

Skills and Experience

  1. Demonstrate effective oral and written communication skills.
  2. Ability to maintain professional and positive relationships with a wide variety of people.
  3. Flexibility and ability to adjust to changing work demands.
  4. Strong problem-solving and time management skills.

Desired

  1. Experience supporting individuals with intellectual disabilities preferably in a 245D licensed service.
  2. Education or training in the human services field.
  3. Experience using technology including but not limited to computers iPads tablets tvs and smartboards.

WORK ENVIRONMENT

This role is based primarily in the community. It may involve outdoor or indoor public spaces with varying noise accessibility and sensory conditions. The role requires adaptability creativity and ongoing communication with program leadership.
