Job Title: Social Worker - Emergency Dept (High Utilization)
The Mount Sinai Hospital is a 1134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical emotional and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907 the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education professional development and extra-curricular opportunities.
The social worker in this position will facilitate care transitions of patients who are identified as frequent utilizers of the emergency department (ED).
Responsibilities include comprehensive psychosocial assessment a full range of psychosocial interventions for patients/families including transitional care; and ongoing efforts to engage patients with high utilization of emergency services after discharge in the community. Services will include counseling including crisis intervention and assistance with concrete services (e.g. pharmacy home care transportation entitlements referral for community services) placement of patients in facilities and close collaboration with social workers in outpatient settings. The social worker in this position will meet with patients in-person in the emergency department setting at Mount Sinai Hospital and will continue to follow post- ED visit enhancing linkages to primary and specialty care and community organizations.
The social worker in this position will also have project management and development responsibilities specific to local and system efforts to respond to frequent utilizers of emergency medicine and inpatient care. Responsibilities will include administrative tasks (organizing and disseminating meeting minutes for system committee meetings updating clinical flags in Epic as well as patient care plans) process mapping as well as data collection and analysis in partnership with supervisor.
Hours: This is a full-time position with the hours of 9a-5p Monday- Friday.
Funding: This position is funded by a time-limited 2-year grant through an outside foundation.
Schedules are subject to change based on Emergency Department and inpatient flow.
