Job Summary

SUMMARY

As a Certified Home Health Aide (HHA) you will provide care to clients in their home following their plan of care. You make a positive difference each and every day by demonstrating care and compassion to our patients.

STATUS:Full Time

LOCATION: Rochester Regional Health Home Care - Newark

DEPARTMENT: HHC LHCSA HHA NEWARK

SCHEDULE: Day/Evening

RESPONSIBILITIES

  • Patient Care & Service. Promote and restore patients health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care light housekeeping meal preparation and errands
  • Communication. Maintain effective communication to convey patient health status treatment plans and progress; communicate with office and supervisory staff regularly
  • Documentation. Accurately and completely document client care on activity sheet
  • Compliance. Adhere to required department and system protocols HIPPA regulations local state federal requirements

EDUCATION:

LICENSES / CERTIFICATIONS:

HHA - Home Health Aide certification - New York State Education Department (NYSED)New York State Education Department (NYSED)New York State Education Department (NYSED) NYS DL - valid New York State Drivers License - New York State Department of Motor Vehicles (NYSDMV)New York State Department of Motor Vehicles (NYSDMV)New York State Department of Motor Vehicles (NYSDMV)

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally and/or 10 to 25 pounds of force frequently and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking standing or squatting.

$18.65 - $19.75

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience relevant qualifications specialty internal equity location and contracts.
