Success Coach for El Puente Program
Families & Youth Innovations Plus (FYI)
Position: Full-Time
Hourly Rate: $17.98 - $21.76 (3 levels based on experience and education
FLSA Status: Hourly/Non-Exempt
Reports To: LVP Program Supervisor
Department: La Vida Project - El Puente Program
Job Summary
The Success Coach provides specialized case management services to individual client needs. They also create programs to benefit clients and educate clients on available community resources. Success coaching ensures that clients receive LVP services in a timely appropriate efficient and coordinated fashion.
What youll Do:
Key Responsibilities
- Provide clients with supportive services
- Coordinate services between providers and clients
- Interpret agency services to clients and community
- Provide case management services and skill-building services to youth in the program
- Provide housing case management services
- Maintain individual case records on clients receiving services
- Provide specialized case management services to individual client needs
- Educate clients on available community resources and services
- Create programs and services to benefit clients and the community
- Attend community meetings relevant to program and clients
- Administer and facilitate the completion of appropriate intake forms assessments intake packet and
- service plans
- Conduct field visits attend meetings and be available to deliver services during traditional and nontraditional
- hours (when needed) in; the clients home educational institutions places of employment FYIs Main office JPPO offices detention facilities reintegration centers or any other setting that enables and promotes the consistent and structured delivery of LVP services
- Organize and facilitate Life Skills Groups as needed in both individual and group settings
- Required to participate and interact in youth activities (for example: camping sport activities overnight youth conferences etc.) which may involve occasional overnight activities and trips
What You Need:
Qualifications and Experience
- Bachelors degree in Social Work Family and Consumer Sciences Criminal Justice PsychologySociology or Criminology from an accredited College or University.
- Must pass a CYFD Criminal Records Check
- Must maintain a full certification of the following trainings; CPR/First Aid and CPI
- Must maintain a valid NM Drivers Licenses.
