Durham Community Health Centre (DCHC)is a registered charitable organization that provides integrated accessible and equitable community-based primary care wellness services and health education to Durham community members who face multiple barriers to their health and well-being.

DCHC also provides equity-based programs and services that focus on priority populations such as Indigenous Black the 2SLGBTQI Community Newcomers to Canada Seniors and Unattached patients (i.e. those without a family doctor) to name a few. DCHC ensures Durham community members receive not only sick care but preventive care as well.

Position Overview
As a critical first point of contact the Centralized Intake Navigator (CIN) is the gateway to DCHCs programs and services. Utilizing their program knowledge skills and judgment the CIN will review and prioritize all referrals respond to program/service inquiries as well as ensure that the client intake/discharge processes are carried out in a timely accurate and sensitive manner. The CIN will also make internal and external referrals as appropriate and act as a linkage for clients and patients across DCHC services and the broader healthcare system.

Key Responsibilities

As the first point of contact for DCHC the CIN will:

  • Work in a fast-paced environment independently and as part of a team.
  • Utilize critical judgment to determine the priority urgency and types of service provision needed for new clients.
  • Ensure referrals for all DCHC services are complete prioritized and classified by the urgency and level of care or support required.
  • Open new client files and complete the centralized intake documentation including demographic and other required information in an accurate manner.
  • Ensure service requests and referrals are monitored so that all clients and families receive timely responses with follow-up calls to determine if the care/service needs have been met.
  • Act as an initial and final point of contact for clients their families and caregivers.
  • Assist with service/care transitions.
  • Utilize effective care coordination and communications strategies as required.
  • Provide DCHC programmatic information to prospective clients referring health care professionals and community members.
  • Provide information about the spectrum of DCHC services to support smooth transitions of care and service from one point of care to the next.
  • Manage client information and assist with ensuring it is up-to-date and meets DCHC documentation policy standards.
  • Participate and attend agency wide committees.
  • Perform other duties as assigned that are reasonable within the scope of the job.

Qualifications

  • Diploma or Degree in Office Administration Social Services or related field from an accredited academic institution.
  • Minimum 2 years experience administrative experience in a healthcare or community setting.
  • Excellent communication relationship-building and advocacy skills
  • Oral and/or written fluency in other languages would be considered an asset.
  • Knowledge and proficiency with Microsoft Office.
  • Working knowledge of electronic records management an asset such as PSS.
  • Demonstrated knowledge of health care systems as it relates to community support services.
  • Demonstrated problem solving and critical thinking.
  • Demonstrated time management and ability to work autonomously.
  • Excellence in communication partnership building and interprofessional care commitment.
  • Demonstrated commitment to continuous learning and professional development.
  • Proficiency with data entry reporting and quality improvement tools.
  • Valid Ontario drivers license and unlimited use of a vehicle is a requirement
  • Flexibility to work some evenings and weekends to support youth access.
  • Satisfactory criminal reference and vulnerable persons check
