Griswold Home Care is seeking a compassionate organized and service-drivenClient Care Coordinatorto join our growing team. This role is essential in ensuring our clients receive the highest quality of care and that our caregivers feel supported and set up for success.
About the Role
The Client Care Coordinator is responsible for managing client relationships overseeing service schedules supporting caregivers and ensuring seamless communication between clients families and our office team. You will be a key part of delivering the exceptional care Griswold is known for.
Responsibilities
Serve as the primary point of contact for new and existing clients
Conduct client intakes assessments and ongoing check-ins
Coordinate caregiver schedules and match caregivers to clients based on needs and skillsets
Support caregivers with updates training reminders and problem-solving
Maintain accurate client records and documentation
Assist in resolving scheduling challenges or client concerns in a timely professional manner
Collaborate with the office team to ensure smooth operations and excellent service
What Were Looking For
Strong communication and customer service skills
Excellent organization and attention to detail
Ability to multitask in a fast-paced environment
Empathy patience and a genuine desire to serve seniors and their families
Previous home care or healthcare coordination experience a plus
Proficiency with scheduling software is helpful (training provided)
