POSITION SUMMARY

The Adult Brain Injury (ABI) Service Coordinators connect individuals aged 21 to 65 who are living with a traumatic brain injury to resources that promote independent living community participation and employment. They may also connect eligible participants to community-based rehabilitation services to help achieve their personal goals.

Essential Duties and Responsibilities

  • Improved Access to Services: Successfully coordinate services for individuals with traumatic brain injuries by conducting comprehensive assessments setting achievable goals and linking participants to appropriate resources resulting in enhanced quality of care and independence.
  • Optimized Resource Utilization: Match participant support needs to community-based rehabilitation services ensuring financial eligibility and maximizing resource efficiency.
  • Goal Achievement and Progress Monitoring: Develop individualized service plans and continuously monitor participant progress making timely adjustments to ensure goals are met and outcomes are improved.
  • Enhanced Stakeholder Support: Provide technical assistance and guidance to individuals families health units schools healthcare providers and other stakeholders leading to better-informed decisions and improved service delivery.
  • Compliance and Accuracy: Maintain precise documentation of all services in accordance with DHSS and agency standards ensuring regulatory compliance and audit readiness.
  • Community Awareness and Engagement: Conduct outreach and educational initiatives to increase awareness of ABI services resulting in greater community engagement and program utilization.
  • Collaborative Service Delivery: Partner with community organizations and providers to deliver comprehensive support fostering integrated care networks and improved participant outcomes.
  • Timely Reporting: Prepare and submit accurate reports and required data to DHSS within established deadlines supporting program accountability and continuous improvement.

Qualifications

  • Education: Associates degree from an accredited college or university.
  • Preferred Experience:
    • Knowledge of traumatic brain injury and related rehabilitation services preferred.
    • Experience in case management service coordination or community health programs desirable.
  • Skills:
    • Strong communication and interpersonal skills.
    • Ability to work independently and manage multiple priorities.
    • Proficiency in documentation and reporting.
    • Familiarity with community resources and public health systems.

Knowledge Skills and Abilities

  • Understanding of TBI and its impact on individuals and families.
  • Competence in providing education and technical assistance to diverse audiences.
  • Commitment to promoting independence and community integration for individuals with brain injuries.

Required Miscellaneous Qualifications

  • Must have reliable transportation and an acceptable driving record. Must use your own vehicle for transportation. Must be able to drive for work-related assignments to individuals homes to meet face-to-face with individuals on caseloads.
  • Must have a valid drivers license.
  • Must provide proof of personal valid vehicle insurance.
  • Must have access to reliable internet for telecommuting employees.

PHYSICAL REQUIREMENTS
