Salary: Level 7 Step 1 ($3410.25 bi-weekly)

Reporting to the Health Programs & Services Manager the Elders Support Worker implements manages and administers the TH Elders Support Program. Services and supports provided by the incumbent include home visits assistance with navigating programs and services advocating for needs arranging travel assisting with medical and other appointments outings meetings and events addressing enquiries and concerns providing explanations and resource information liaising with family members planning social cultural and recreation activities providing logistical support to the Elders Council assisting with implementing strategies to manage chronic conditions and serves as a member of the Wellness case management team to ensure that Elders receive consistent timely and quality supports.
An Eligibility List may be established from this competition the duration of the eligibility list may be up to 12 months. The eligibility list may be used to fill future permanent full-time permanent part-time temporary full-time temporary part-time term or auxiliary on-call vacancies within the same department and classification based on the organizational needs by going the next highest ranked candidate until the eligibility list expires.

Essential Qualifications:
- Grade 12 (or equivalent) combined with a post-secondary diploma in a social- or health care-related discipline along with a minimum of two years of experience in providing programs services and supports to Elders. An equivalent combination of education training skills and experience may be considered.
- Knowledge of current practices in health promotion basic chronic disease prevention and management and community health care support programs and services.
- Knowledge of Elder benefits and financial assistance programs.
- Knowledge of (or willingness to learn) TH government structures culture traditions history demographics family structures socio-economic dynamics challenges and aspirations basic phrases of the Hän language and the intergenerational impact of residential schools on individuals families and communities.
- Proficiency in Microsoft Office/365 (Word Excel Outlook PowerPoint) SharePoint and videoconferencing technologies.
- Experience with case management planning.
- Experience in developing presentations and facilitating workshops.
- Experience in conflict management and dispute resolution.
Assets:
- Excellent planning organizing research analytical and time management skills.
- Strong program and event planning implementation
and evaluation
- Experience with developing and administering strategic plans budgets and workplans.
Conditions of Employment:
- Criminal Record Check with Vulnerable Sector Screening.
- Valid Class 5 Drivers license and willingness to obtain a Class 4 (if requested).
- Standard First Aid and Remote/Wilderness First Aid training (or willingness to obtain).
- TH101 Cultural Awareness training.
