Job Summary
Responsible for collaborating with the healthcare team to assess patient needs develop care plans and connect individuals with appropriate community resources. This role provides counseling advocacy and coordination of services to improve the overall well-being of patients and promote successful healthcare outcomes.

Essential Functions
-Conduct comprehensive assessments of patients social emotional and practical needs to identify areas requiring intervention or support.
-Collaborate with the healthcare team to develop individualized care plans that address patients psychosocial concerns and align with their healthcare goals.
-Assist in identifying and addressing barriers to care such as financial limitations transportation issues or lack of social support.
-Provide counseling and emotional support to patients families and caregivers dealing with healthcare-related challenges such as chronic illness terminal conditions or caregiving stress.
-Offer active listening empathy and guidance to help individuals cope with their emotions make informed decisions and enhance their overall well-being.
Education
Bachelors Degree Social Work required
