Job Title: RN Admission Nurse
Department: Mozaic Home Care
Supervisor: Clinical Director
Job Objective:
The RN/Admission Nurse is responsible for the initial assessment and admission of the home care patient and the development of the initial plan of care (POC). Responsible for the observation assessment nursing diagnosis counsel care and health teaching of patients and their caregivers.
Minimum Qualifications:
Education: Degree in Nursing (AD or BSN)
Experience: 1 year in a clinical setting home care experience preferred
Licensure: Registered Nurse valid in CT
Essential Job Functions:
- Admission of patients for service including the comprehensive assessment
- Medication reconciliation
- Accurate completion of OASIS assessment
- Development of Initial Plan of Care (POC) including visit frequency
- Communication and coordination with primary care physician to establish POC.
- Health teaching regarding medications equipment and disease process/management with patient and caregiver
- Communication with other members of the care team (i.e. clinical supervisors therapists aide supervisor) to ensure effective hand-off for ongoing care.
- Order necessary supplies for timely and effective care
Physical and Mental Requirements:
- Ability to sit or stand for long periods of time.
- Ability to speak clearly and be understood.
- Able to hear sounds within some background of noise as in answering the phone intercom etc.
- Visual acuity to see computer and read printed documents.
- Ability to concentrate on fine details with interruptions.
- Ability to attend to tasks/functions for more than 60 minutes at a time.
- Ability to understand and relate to the reasons behind several related concepts.
- Ability to remember multiple tasks /assignments given to self and others over long periods of time.
- Ability to perform gross body coordination such as walking stooping filing etc.
- Ability to perform tasks which require hand eye coordination such as keyboard skills writing or picking up paper.
Environmental Conditions:
- Rare exposure to blood body tissue or fluids.
- Possible exposure to high/low temperatures.
