Role and Responsibilities:

A qualified professional who directs supervises evaluates and provides physical therapy services to adult patients in the home following physicians orders.

Essential Functions:

Evaluate patient using diagnostic muscle nerve jointand functional abilitytestsand measures.
Prepare an assessment of the patients condition at the time of theinitialevaluation and perform periodic re-evaluations develop a written problem list and plan of care including types and frequencies of treatment.
Recommend modification of plan of careto physicianas needed.
Document clinical notes on the date of the visit into medical record and keep all records and summariesup-to-date.
Treat patients to address impairments and functional limitations with a goal ofreachingmaximum functional independence.
Instruct and/or recommend patients and their caregivers in the use of prosthetic orthoticand durable medical equipment (canes walkers wheelchairs etc.).
Include and educate thepatientandcaregiver(s)regardingtreatmentplanand goals.
Attend physical therapy staff meetings and in-service presentations.
Participate in case conferences.
Participate in the development and periodic revision of the physicians Plan of Treatment.
Assureappropriate safetypractices and maintenance of equipment.
Other duties as assigned.

Education Requirements:

Graduateof an accreditedPhysicalTherapy program.

Qualifications and Requirements:

Current valid Physical Therapy licenseissuedbyNYSDepartment ofEducation.
One year of practical experience as a licensed physical therapist.

Work Environment:

The Physical Therapist is primarily ina non-officesetting and may be exposed to outdoor conditions.

The working conditions are classified as medium work:

Medium work - Exerting up to 50 pounds of force occasionally and/or a negligible amount of forcefrequentlyor constantly to lift carry push pull or otherwise move objects including the human body. Medium work involves sitting and standing.
