Asa local outreach associateyou will contacttarget patients explain the P3 Health Partner Programto patientsor their contactsand schedule these patients for visits in a variety of mayassistin rescheduling with the P3 teams will ensure smooth transitionsthroughout the patients health care journey.

Essential Functions

• Perform telephonic outreach to patients following astandardized operational process.
• Work collaboratively withP3 Health Partnerteamsand their affiliates.
• Update andmaintainaccuratepatient contact information.
• Ensure patients with which youinteracthave a solid understanding of the P3Health Partner offerings as well as a continued relationship with their primary care physicians.
• Accuratelyschedulepatients for visitsina variety of locations.
• Adhere to the local outreach role-based handbook making recommendations for improvement whenidentified.
• Serve as a positive and collaborative team member.
• Upholdhigh standardsofexcellenceand professionalism.
• Respond positively to make any necessary adjustments tocase reviews.
• Confidently usetechnology topositivelyimpactoutcomes.
• Ensure patient privacy and adherence to all HIPAA regulations and standards for handling PHI.
• Participate in quality improvement initiatives and operational efficiency.
• Maintain compliance with requiredtrainings meeting attendancetimelyresponses toemailsand other requirements.

Qualifications

• High school completion
• Experiencein a service industry rolepreferred
