The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and interdisciplinary team collaboration. Strong communication and collaboration with patients providers and team members are essential.

Responsibilities:
- Provide skilled nursing care to patients.
- Conduct patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Communicate effectively with patients providers and team members.
- Perform other related duties as assigned.

Required Skills & Certifications:
- Active Registered Nurse license.
- BLS certification.
- NRP certification.
- Minimum 1 year of experience.

Preferred Skills & Certifications:
- Specialities in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room.
- Clinical Assessment Medication Administration and Treatment Planning skills.
- Two years minimum of hospital home health or hospice care experience.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Bachelors degree in Nursing or related field.
- Drivers license car insurance and automobile for field work.
- Specialities in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room.
- Clinical Assessment Medication Administration and Treatment Planning skills.
- Two years minimum of hospital home health or hospice care experience.

Special Considerations:
- May be required to float between like units.
- On-call requirements are pending.
- RTO must be included at the time of submission.
- The facility is able to call off once per two-week pay period.
