Job Summary

Community Based Services Coordinator

Position Objective:

The Community Based Services Coordinator works with and to support clients over the age of 65 years to remain in their home and be as independent as possible for as long as possible. The role is client-centred and based on the Active Service Model. The Home Services Coordinator will work alongside the NDIS Coordinator (supports clients under 65 years) as part of a multi-disciplinary Community Services team to ensure participants are supported with a suite of flexible quality services targeted to meet individual needs and expectations.

Selection Criteria:

Essential:

Diploma qualification in health nursing community services or aged care (preferred)
Understanding of the Aged Care Sector with particular skills and knowledge of Commonwealth Home Support Program and Support at Home Packages.
Competence in advance health assessment health coaching and a detailed understanding of health determinants contemporary palliative care practices and chronic disease management
The ability to build working relationships with clients and networks within the community
Demonstrated ability to liaise consult and negotiate with a wide range of stakeholders including community members clients and their carers and work within a broader multi-disciplinary team
Demonstrated capacity to provide leadership including case management care coordination and advocacy for clients
Experience and competence in the use of the Microsoft Office Suite
Current 1^st Aid Certificate

Desirable:

Knowledge of and ability to manage human economic/financial and physical resources
Demonstrated ability to lead continuous improvement programs for the CBS services
Proven understanding of the role and value of healthcare organisations in a rural environment
Valid Victorian or NSW Drivers License

Key Responsibilities:

Manage and coordinate service requests for people over 65 years in the local community working with a multidisciplinary team as part of the Continuum of Care program
Design and implement bespoke Commonwealth Home Support Program and Support at Home Packages /care plans in consultation with the client and/or carer and undertake planning and coordination as appropriate
Coordinate the management of client data and reporting through the Corryong Health eTools program to comply with CHSP funding requirements
Complete client reviews as determined within the agreed care plan and as required or if requested by the client/carer
Manage and schedule the allocation of community support staff rosters staff leave training and meetings liaising with the NDIS
Work within an Active Service Model of Care that focuses on promoting capacity building wellness and restorative care.
Develop implement and monitor CBS policies and procedures
