DescriptionJob Title: Social Worker - Adolescent Alternative Day Program - Mount Sinai Morningside - Full Time - Day
- Assesses and evaluates patients/families social psychosocial and environmental situations to determine patient needs.
- Demonstrates knowledge of the disease entity and common medical interventions in the service area assigned.
Responsibilities- Develop options and treatment plans in a timely manner.
- Provide direct service to patients and families selecting treatment modality casework technique compatible with assessment and identified goals.
- Ability to identify select access and make referrals to appropriate community resources.
- Monitor coordinate manage activities for high risk psychosocial patients to ensure implementation of multiple services.
- Advocate for patients and families by interpreting their needs to and eliciting cooperation from other disciplines and community agencies.
- Identify and assist team in managing obstacles to treatment and/or discharge plans.
- Provide psycho-education for patients and families.
- Knowledge of child protection elder abuse domestic violence policies and protocols.
- Participate interdisciplinary rounds case conferences staff meetings.
- Document in the medical record in accordance with standards set by Department of Social Work and/or regulatory agencies.
- May supervise social work interns.
- Participate in various outreach and community-based activities/interventions to elicit referrals and support engagement.
Qualifications- Masters in Social Work from an accredited school of social work. Knowledge of social work services theories techniques community resources and agencies.
- Related experience in the social service and/or health care field preferably one year in a medical setting which can include related field placement.
- SIFI trained.
- LCSW preferred; LMSW post exam and with 1-2 years clinical experience may be considered if the following experience is present:
DBT experience
CBT experience
MET experience
Treatment of co-occurring mental health and substance use disorders with adolescents. - Treatment of high risk patients.
- At least 1 year experience with adolescents conducting individual therapy running therapy groups conducting milieu therapy from a DBT and harm reduction approach.

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $44.35 - $57.11 Hourly. Actual salaries depend on a variety of factors including experience education and operational need.
