Job Description
We are currently seeking a Practice Nurse to join our supportive and well-organised team on a part-time basis. Located in Merseyside.
Days: 3-5 days - working pattern negotiable.
Hours: 2030 hours per week
About the Role
As a Practice Nurse you will be responsible for delivering high-quality care across various clinical areas including chronic disease management and health promotion.
Duties may include:
- Running chronic disease clinics such as:
- Annual reviews for diabetes asthma and COPD
- Hypertension reviews
- Cervical smears ECGs urinalysis phlebotomy and wound care
- Suture/clip removal
- Contraception advice
- Vaccinations routine travel seasonal (adults and children)
- New patient health checks and NHS health checks
- Providing health education and advice
- Occasionally undertaking home visits to carry out reviews and other nursing duties for housebound patients

- A modern purpose-built facility with ample on-site parking
- A collaborative environment working closely with local practices as part of a Primary Care Network (PCN)
- Rated Good in all areas by the CQC
- High QOF achievement
- Clinical system: EMIS Web
- Practice is open Monday to Friday 08:0018:30
Additional Responsibilities
- Maintain accurate and up-to-date clinical records
- Collaborate with the HCA and Phlebotomy team
- Diagnose and manage care for patients in the clinic or at home
- Promote opportunistic health education
- Manage stock control and infection prevention procedures
