We are looking for a committed Care Coordinator to provide culturally safe care coordination and wellbeing support to community members living with mild to moderate mental health needs. Working as part of a connected multidisciplinary team you will help improve access to services build stronger referral pathways and promote holistic health and wellbeing.

About the Role
- Coordinate culturally safe mental health care for Aboriginal and Torres Strait Islander people
- Support access to mental health services GPs and community providers
- Deliver individual and group wellbeing education
- Strengthen collaboration between health and community services
- Build relationships with communities to support culturally appropriate care
- Travel weekly within the region to outreach locations
About You
- Identify as an Aboriginal or Torres Strait Islander person (genuine occupational qualification under s14 of the Anti-Discrimination Act 1977 NSW) you may be asked to provide proof of nationality.
- Certificate IV in Mental Health or equivalent
- Experience engaging with Aboriginal communities in a respectful culturally informed way
- Ability to work independently and within a team
- Strong communication relationship-building and organisational skills
- Current driver licence WWCC/Blue Card and National Police Check
