What to expect.
The Behavioral Health Care Manager position is formasters-level licensed clinicians. This position will join outpatient teams as a behavioral health care manager with Sheppard Pratts Collaborative Care Program at an assigned practice location. The Behavioral Health Care Manager will provide clinical care in collaboration with primary care providers and consulting psychiatrists. Care will consist of brief counseling coordinating treatment and providing proactive follow-up to treatment response utilizing a collaborative care approach.
Specific Responsibilities:
- Outreachto patients referred by primary care discussing the service and scheduling.
- Perform comprehensive psychosocial intake.
- Develop coordinate and update patients treatment plans.
- Provide patients with brief evidence-based psychotherapysuch as cognitive behavioral therapy motivational interviewing behavioral activation and problem-solving treatment.
- Support medication management including consultation with psychiatrist.
- Conduct proactive follow-upto assess and document patient response to treatment.
- Attend team meetingsas required by the Program Manager.
- Develop working relationshipswith primary care providers in practice to collaborate about patients and support knowledge of the current state of Collaborative Care services.

What we need from you:
The pay range for this position is $65000 minimum to $83000 maximum. Pay for this position is determined on a number of factors including but not limited to board certifications and years and level of related experience. Care provided in both virtual/telehealth and face-to-face visit formats based on the needs of the client and host practices. Behavioral Health Care Managers may telecommute up to 3 days per week with 2 days on-site.
