Hours Full Time 40 Hours per week
Job Objective
Responsible for ensuring safe environment of care for adult family living clients through caregiver coaching and support including orientation and continuing education in the provision of daily care.
Minimum Qualifications
Education: Graduate of an accredited nursing school; Registered or Licensed Practical Nurse. Bilingual (Spanish or Creole) a plus.
Experience: Minimum two (2) years of nursing experience case management with aging adults or adults with disabilities. Preferred experience in community-based setting.
Licensure: State of CT Nursing License; Valid CT Moter Vehicle Drivers License and state required automobile insurance and registration.
Essential Job Functions
- Represents organization with highest quality of professionalism and expertise in responding to referrals or inquiries from prospective caregivers consumers and professional partners to encourage potential consumers to choose Mozaic as their AFL provider agency.
- Completes intake and orientation of new AFL caregivers within scope of nursing practice. Works with partner organization teams (access agency or other state agencies) to execute state-set plan of care.
- Coordinates referral to other disciplines as appropriate to each case needs.
- Conducts home visit on day of enrollment and conducts additional home visits as indicated and in compliance with state regulatory requirements and Mozaic AFL standards.
- Provides on-going coaching training and support of caregiver.
- Records all notes in a timely fashion in accordance with program standards.
- Reviews weekly care report from caregiver and appropriately addresses changes in the consumers condition or environment.
- Reports changes to access agency care manager or directs caregiver when to report changes as needed to physicians or emergency services.
- Ensures compliance with accepted professional standards and organization/local/state/federal regulations.
- RN oversees AFL LPN nurse.
- All other duties as assigned.
