Where youll be working
Youll be based on Wonnarua Country at Cessnock District Hospital working out of a shared office with a tight-knit family-like team. Youll be side by side with an Aboriginal Health Practitioner and Aboriginal Health NP plus a chronic disease RN whos always up for a yarn and a problem-solve. The wider ICCAPP crew has your back too offering extra support guidance and connection whenever you need it. Its the kind of team where you never feel like youre doing things alone.
What the role looks like
Your days will take you into the community supporting Aboriginal people across Cessnock Muswellbrook Singleton and Maitland. Youll be the expert nurse who brings chronic care out of the hospital and into peoples lives. Expect a mix of clinical assessments case management care coordination and working closely with local GPs and primary health networks to keep care flowing smoothly.
No two days are the same. One morning you might be reviewing a care plan with a patient recently discharged from the afternoon you might be chatting with a GP about a complex case or linking someone with the services that help them stay on track. Its meaningful work with a personal touch supporting mob to manage chronic disease in a way thats culturally respectful practical and genuinely empowering.
Requirements:
Registered Nurse with current Authority to Practice with AHPRA
5 years full time equivalent post registration experience with at least 3 years full time equivalent experience in this specialty addition the employee must have approved postgraduate nursing qualifications in chronic disease management
