- Conduct comprehensive patient assessments in home facility and community settings identifying evolving needs and goals
- Skillfully manage complex symptoms optimize comfort measures and adjust medications within care plans
- Collaborate with physicians social workers chaplains and other interdisciplinary team members to coordinate holistic care
- Educate patients and families about symptom management advance care planning and endoflife decision making
- Develop document and revise individualized care plans aligned with patient goals and cultural preferences
- Monitor safety infection control and regulatory compliance while maintaining high standards of quality and ethics
- Provide bereavement support resources and guidance to caregivers and families as care transitions
- Engage in ongoing professional development quality improvement and opportunities to mentor peers
