Home Health RN Responsibilities
Provide skilled nursing care in the patients home setting includingassessments treatments wound care and medication management.
Develop and implement individualizedhome health care plans adjusting as patients conditions change.
Educate patients and families on health conditions treatments and at-home care.
Collaborate with theDirector of Nursing physicians and interdisciplinary teamsto coordinate quality care.
Qualifications for Home Health RN Jobs
Graduate of an accredited nursing program (BSN preferred).
Current California Registered Nurse (RN) license in good standing.
At least 1 year of professional nursing experience (home health nursing experience strongly preferred).
Public Health Nurse (PHN) certification a plus.
Strong clinical judgment communication and organizational skills.
Valid drivers license reliable transportation and ability to travel to patient homes in Inglewood Hollywood Brentwood Santa Monicaand surrounding Los Angeles County areas.
