Role Specifics and Benefits: As an RN in LTC youll deliver comprehensive nursing care for residents with chronic conditions post-acute needs and age-related health concerns. Your responsibilities include: conducting comprehensive assessments and ongoing monitoring; administering medications and therapies in accordance with physician orders; developing and updating individualized care plans in collaboration with physicians therapists social workers families and the residents themselves; managing wound care IV therapy catheter care and other routine and complex interventions; supporting mobility activities of daily living nutrition hydration and sleep; implementing fall-prevention strategies and safety protocols; providing patient and family education on disease management medications and discharge planning; participating in interdisciplinary rounds and family conferences; documenting accurately in electronic health records ensuring compliance with regulatory standards and contributing to quality improvement initiatives.
