Requirements:
- Active New York RN license
- Nurse Compact / Multi-state license (required)
- Minimum 3 years of diabetic care experience
- Experience in care coordination or case management (preferred)
- Must provide own technology (computer webcam microphone speakers phone stable internet)
- Strong communication critical thinking and organizational skills
Responsibilities:
- Provide remote case management for diabetic patients
- Conduct virtual assessments and ongoing monitoring
- Develop implement and modify individualized care plans
- Collaborate with multidisciplinary healthcare teams
- Document all patient interactions per facility standards
- Ensure compliance with state regulations and organizational policies
- Maintain confidentiality and secure handling of patient data
