About the role

As a Community Connector youll be working directly with vulnerable older people facing difficult circumstances such as social isolation homelessness mental health issues substance use family and domestic violence or justice system involvement. Youll build relationships with individuals in the community listening to their concerns without judgement and supporting them to access aged care and essential services. Youll collaborate with local services and networks to strengthen community responses to ensure all older people within your designated region can access the support they need.

Key responsibilities include:

  • Deliver high-quality specialist services through information sharing support representation and assertive outreach.
  • Provide individualised and intensive support to clients to ensure they understand and can access relevant aged care and other services.
  • Engage with community supports and stakeholders to promote integration of health aged care and other systems at the local level.
  • Check-in with clients on a periodic basis and provide follow-up support once services have commenced.
  • Support clients to interact with My Aged Care and relevant assessment services.
  • Promote ADA Link services by networking with other community organisations.
  • Maintain high standards of documentation and consultation.
  • Liaise with other care finder services and support groups for older people.
  • Forge strategic partnerships with diverse cultural and community groups.

About you

  • Relevant tertiary qualification or equivalent experience in human services social work or community services.
  • Demonstrated ability to build respectful trust-based relationships with vulnerable older people or disadvantaged groups.
  • Understanding of and ability to work within a social justice framework.
  • Ability to understand and apply legislation policies and procedures.
  • Proven ability to problem-solve and find solutions for clients in complex circumstances.
  • Excellent communication presentation and community engagement skills.
  • High level computer skills Microsoft Office Suite or equivalent.
  • Commitment to continuous quality improvement.

Essential requirements

  • Valid Australian Drivers licence.
  • Current (less than three (3) years old) National Criminal History Certificate.
  • Valid NDIS Worker Screening Clearance.
  • Valid Working with Children/Blue Card.
