ESSENTIAL FUNCTIONS: The listed functions are illustrative only and are not intended to describe every function which may be performed in the job level.


  • Responsible for individualized case planningdischarge planning and monitoring progress.
  • Responsible for case management duties to include planning and implementing all facets of programming and associated documentation.
  • Prepares organizes schedules and implements psychoeducational groups and interventions.
  • Monitors patient compliance with established goals and objectives; updates case plans as needed.

Qualifications


PREFERENCES:


Preference will be given to qualified applicants who are currently a certified Social Worker.


KNOWLEDGE:


  • Knowledge of human behavior motivation and interpersonal behavior.
  • Knowledge of the varied techniques of programming/education.
  • Knowledge of social work principles and practices.
  • Knowledge of interviewing and counseling techniques.
  • Knowledge of current social economic and health problems and community resources.


MINIMUM QUALIFICATIONS:


Education:
Bachelors Degree (typically in Social Services)
Experience:
0-2 years of progressive work experience (typically in Casework)
OR
Education & ExperienceSubstitution:
3-5 years of progressive work experience (typically in Casework)
Certificates Licenses Registrations:
None

Necessary Special Requirements


PHYSICAL WORKING CONDITIONS:


  • Physical ability to lift twist bend pull and push up to 50 lbs.


NOTES:


  • FLSA: Non-Exempt
  • Pre-employment drug and alcohol testing will be conducted on safety-sensitive positions. Drug testing will be completed on any position if reasonable suspicion exists.
  • A background check including the taking of fingerprints will be completed through the Wyoming Department of Criminal Investigation the Department of Family Services and the Federal Bureau of Investigation for every successful applicant.
