The Clinical Nurse Lead Primary & Aged Care provides clinical leadership assessment and risk oversight across CHSP HACC-PYP and Support at Home to ensure safe culturally grounded care. The role delivers clinical triage comprehensive assessments clinical risk escalation and care planning aligned with aged care reforms and Aboriginal Primary Health guidelines.

Position Purpose
The Clinical Nurse Lead Primary & Aged Care provides clinical leadership clinical assessment risk management and clinical oversight for all clients who receive clinical services through GEGACs Commonwealth Home Support Programme (CHSP) the Home and Community Care Program for Younger People (HACC-PYP) and the Support at Home Program.
The role ensures that clients across these three program streams receive safe culturally grounded high-quality clinical care including clinical triage comprehensive health assessments (including appropriate Medicare-rebatable assessments) identification and escalation of clinical risks development and monitoring of clinical care plans and alignment with the Strengthened Aged Care Quality Standards aged care reforms Support at Home transition requirements and GEGACs clinical governance framework.
The Clinical Nurse Lead works closely with Care Partners Home Care Support Workers the Care Partner Lead the Social Inclusion Lead and Lifestyle Workers to provide day-to-day clinical guidance (non-managerial) ensuring clear clinical boundaries safe practice early intervention health promotion and culturally respectful care for Aboriginal clients.
Key Selection Criteria
- Demonstrated clinical assessment capability: Proven experience conducting comprehensive clinical assessments across primary health care and/or aged care settings including functional cognitive mobility frailty chronic disease and deterioration assessments.
- Advanced clinical risk assessment & decision-making: Strong ability to identify interpret and escalate clinical risks apply clinical reasoning recognise early warning signs of deterioration and implement safe evidence-based responses.
- Experience in multidisciplinary community-based care: Demonstrated capability working within community settings and collaborating effectively with multidisciplinary teams including Care Coordinators Aboriginal Health Workers Allied Health GPs cultural teams and social/support staff.
- Knowledge of Aged Care reforms safeguarding & standards: Sound understanding of the Strengthened Aged Care Quality Standards the new Aged Care Act Support at Home requirements and SIRS obligations including their clinical implications for safe practice.
- Chronic disease management expertise: Experience supporting Elders and older adults living with chronic conditions with the ability to recognise exacerbations interpret clinical needs and support culturally safe self-management and wellbeing strategies.
- High-level clinical documentation & governance skills: Strong capability in clinical documentation use of electronic clinical systems care planning incident reporting and alignment with RACGP and Aged Care governance frameworks.
- Clinical guidance within a diverse multidisciplinary team: Ability to provide clinical advice mentoring and support to staff performing cultural social community support and care coordination roleswithin a culturally respectful and collaborative environment.
- Cultural safety trauma-informed practice & Aboriginal health: Demonstrated understanding of cultural safety Aboriginal community-controlled health principles trauma-informed care and Aboriginal concepts of holistic health wellbeing connection to Country and kinship.
- Communication collaboration & professional relationships: Highly developed verbal and written communication skills with the ability to build strong relationships support effective teamwork influence safe practice and engage respectfully with Elders families and community.
- Commitment to continuous improvement: Ability to contribute to service development clinical quality improvement and culturally informed governance activities in a dynamic and evolving service environment.
Qualifications and Registrations Requirement (Essential or Desirable).
- A Bachelor of Nursing or equivalent tertiary qualification with current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA). ESSENTIAL
- Minimum 35 years post-registration clinical experience ESSENTIAL
- Postgraduate qualification (Graduate Certificate Graduate Diploma or Masters) in aged care primary health care chronic disease community nursing or a related field HIGHLY DESIRABLE
- Valid Drivers Licence: A current and unrestricted Victorian Drivers Licence is essential as the role involves regular travel across the Gippsland and East Gippsland region to engage with community staff service sites and external stakeholders. ESSENTIAL
- A satisfactory Victorian Working with Childrens Check and a satisfactory National Police Check (Aged Care / Vulnerable Persons Sector) and NDIS Worker Screening Clearance ESSENTIAL
- Ability to meet occupational vaccination requirements for workers supporting vulnerable people including providing evidence of immunisation history and recommended vaccinations and proof of COVID-19 vaccination to align with GEGAC requirements ESSENTIAL
- A person of Aboriginal and or Torres Strait Islander background DESIRABLE
