What You Will Do:
The Senior Health Center is a memory diagnostic clinic that provides short-term wrap around care to patients (65 and older) and their families after diagnosis. The Senior Health Nurse Practitioner works collaboratively with the medical director and other providers and renders advanced clinical practice services to adults 65 and older. Responsibilities include: assessment/vital signs diagnoses medication review/management and treatment and providing education and support for those coping with varying degrees of cognitive impairment and associated behaviors.
Requirements
- Masters Degree in Nursing and Certification in Family Practice or Adult Primary Care. Gerontology and/or Behavioral health experience or certification preferred.
- Licensed Nurse Practitioner Required
- Certifications: NCCPA DEA CPR (BLS and/or ACLS)
