Greenfield Rehabilitation Agency is seeking a motivated and highly engaged Senior Community Coordinator (PT or PTA) to support our senior living communities and skilled nursing facilities in Delavan WI and the surrounding areas.
This unique role blends direct patient care team engagement and community relationship-building making it the perfect fit for a therapist who enjoys both hands-on practice and leadership influence within the senior care setting.
Position Overview
In this role you will:
Provide individualized therapy services to aging adults within assigned senior communities
Support mentor and collaborate with therapy teams across multiple sites
Serve as the primary contact for facility administrators and partners
Help strengthen relationships and promote Greenfield Rehabs mission and values
Work closely with the Regional Director to ensure clinical excellence team satisfaction and smooth daily operations
This is a high-impact hybrid clinical/leadership opportunity ideal for a PT or PTA who enjoys autonomy communication and building strong care environments for seniors.
Qualifications
Licensed Physical Therapist or Physical Therapist Assistant in the State of Wisconsin
Experience in senior care settings (ALF SNF Memory Care or Home Health) strongly preferred
Strong communication leadership and organizational skills
Ability to support multiple communities and foster positive relationships
Minimum two years of experience preferred (PT or PTA) but strong candidates with less will be considered

- Graduate of an accredited school with a minimum of a bachelors degree in Physical Therapy or associates degree as a Physical Therapist Assistant.
- Certification or license as required by the State of Wisconsin or applicable state to practice as a Physical Therapist or Physical Therapist Assistant
- At least 1 years clinical experience
