About the role

Provide comprehensive Occupational Therapy assessment and evidence-based treatment for clients on the Transitional Aged Care Program in the community.
Identify patients occupational needs through standardised and functional assessments designing tailored patient-centred interventions focused on therapy discharge planning and efficient patient flow.
Manage cases effectively liaising with community services healthcare providers and support networks to ensure coordinated care and appropriate referrals.
Actively contribute to quality improvement and research initiatives within the Transitional Aged Care Program.
Educate the Transitional Aged Care Team and other community groups while responsibly managing occupational therapy equipment and organisational resources.

About you

Youre a qualified and AHPRA-registered Occupational Therapist with a Full Class C Drivers License
You bring proven experience in community settings and have a passion for delivering compassionate care to our vulnerable populations.
Confident in applying clinical reasoning and exercising sound professional judgment in a fast-paced multidisciplinary environment
A natural communicator who thrives in a team building strong relationships with patients families and colleagues alike
Committed to continuous improvement with a demonstrated history of contributing to quality initiatives and evidence-based practice
